Most people who come to us about weight have already lost it before, more than once. The pattern is familiar: something works for a few months, the scale stalls, and the weight comes back with interest. What changed in the last few years is that we can now see, and treat, more of the machinery underneath that pattern instead of arguing about willpower.

This program is run by Dr. Bismah Irfan, MD, who is board certified in Internal Medicine and Nephrology and separately in Lifestyle Medicine. That combination matters here more than it sounds: metabolic weight gain sits at the intersection of thyroid function, insulin resistance, kidney and liver health, sleep, and medication side effects, and all of those need a physician rather than a coach.

Where GLP-1 medications fit, and where they do not

Semaglutide and tirzepatide changed the field, and they are part of what we offer. They are also prescription medications that require evaluation, screening, and monitoring, which is the part that gets lost in the marketing.

Before anything is prescribed, we look at your history and labs: thyroid panel, fasting insulin and glucose or A1c, a metabolic panel covering kidney and liver function, and lipids. Personal or family history of medullary thyroid carcinoma or MEN2 rules these medications out. So does a history of pancreatitis, in most cases. Gallbladder disease, current pregnancy or plans to conceive, and certain gastrointestinal conditions all change the decision.

If a GLP-1 is appropriate, dosing starts low and escalates on a schedule, because most of the side effects people quit over come from escalating too fast. We monitor as you go and adjust. If it is not appropriate, we say so and build the plan without it, which happens more often than you might expect.

Why the medication alone is not the program

A GLP-1 reduces appetite. It does not decide what you eat when you are hungry, protect your muscle mass while you lose weight, or fix the sleep debt and stress load that drove the insulin resistance in the first place. Patients who treat the injection as the whole plan tend to lose weight quickly, lose a meaningful share of it as lean mass, and regain it when they stop.

So the program runs alongside the prescription: protein targets set against your body composition rather than your weight, resistance training, sleep and stress work, and correction of the deficiencies that show up on labs. When the scale stops moving

Plateaus are the most common reason people give up, and they are usually explainable. What the first months look like

You start with a consultation and lab work, not a prescription. Once results are back, Dr. Irfan builds the plan and, if indicated, starts medication at the lowest dose. Follow-up visits track weight, body composition where available, symptoms, and repeat labs on a schedule that depends on what we are treating.

How long people stay in the program varies. Some use medication for a defined stretch and transition off with a maintenance plan. Others have metabolic conditions that call for longer treatment. That is a clinical decision reviewed as you go, not a package decided on day one.

Who this program is not for

We are not the right fit if you are looking for a prescription without an evaluation, or for compounded medication ordered online without labs and follow-up. We also will not treat weight in isolation when the history points somewhere else first, such as untreated sleep apnea, an unmanaged thyroid condition, or an eating disorder, all of which need to be addressed before or alongside any weight intervention.
