iVitality MD is committed to protecting the privacy of your protected health information (“PHI”) in accordance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and applicable Texas law.

How We May Use and Disclose Your Health Information

  • Treatment. We use your health information to provide and coordinate your care, including sharing it with other providers involved in your treatment.
  • Payment. We may use and disclose your information to bill and obtain payment for the services we provide.
  • Health Care Operations. We may use your information for internal operations such as quality assessment, staff review, and administrative functions.
  • As Required by Law. We will disclose information when required by federal, state, or local law.

Uses and Disclosures That Require Your Authorization

Most uses and disclosures of psychotherapy notes, uses and disclosures for marketing purposes, and the sale of PHI require your written authorization. You may revoke an authorization in writing at any time.

Your Rights Regarding Your Health Information

  • Right to inspect and copy your health and billing records.
  • Right to request an amendment if you believe information is incorrect or incomplete.
  • Right to an accounting of certain disclosures we have made.
  • Right to request restrictions on certain uses and disclosures.
  • Right to request confidential communications by alternative means or at an alternative location.
  • Right to a paper copy of this notice upon request.

We are required by law to maintain the privacy of your PHI, to provide you with this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect. We will notify you if a breach occurs that may have compromised the privacy or security of your information.
